Article 301 — Institution of proceedings

(1). Any person legally e!'titled to institute proceedings under this CIVIL PROCEDURE CODE 97 Chapter may, on payment of the prescribed court fee, file a writ- ten, dated and signed application within the tim~ fixed by the law under which the. application is made, or. where no sum time is fixed, within fifteen days from the occurence of the facts on which the application is based. (2). The application shall specify the capacity in which the applicant acts and the provision of the law under which it is made am shall be supported by an affidavit stating the reasons for the application. (3). The applicant shall attach to the application such documents as are required under the following Articles and may attach thereto such other documentary evidence as he deems necessary for the determination of the application.
